Penile lesions
Despite its external location, its mobility and elasticity explain that a penis injury rarely occurs. However, the penis is an organ that can be injured during sexual activity (rupture of frenulum or cavernous bodies) or by external agents (aggressions, bladed weapon or fire, bite, excision or even amputation). A penis injury or trauma can be accidental or deliberate. The most frequent deliberate injuries to the penis are the following
Penile Fracture
This situation occurs during intercourse, in which a short and taut frenulum tears off completely or partially, producing pain and even a noticeable hemorrhage. The treatment consists of surgical suturing (the operation also serves to lengthen the frenulum) with local anesthesia, being able to compress and occlude the area until this happens.
Pen fracture
The fracture of the penis occurs when an abnormal and tangential force is applied in a state of erection (generally during coitus, when the couple is positioned on top of each other). This fracture represents a tear in the tunica albuginea, a thick fibrous layer that surrounds the corpora cavernosa (the two cylinders that house the erectile tissue of the penis). Most cases (75%) occur in a single corpus cavernosum, 25% affect both sides, and in 10%, the injury extends to the urethra.
Symptoms of penis fracture
* An audible creaking sound, followed by loss of erection
* Pain
* Hematoma in the area, affecting the entire penis to its root
If the urethra is damaged, it may produce bleeding, which appears through the meatus (external orifice of the urethra).
During examination, a defect or rupture can be palpated in the affected area and an inward curvature towards the opposite side of the lesion. Ultrasonography (7 MHz probe) serves to examine the integrity of the albuginea layer, also seeing the hematoma. In the face of minimal diagnostic doubt, one should resort to cavernosography, which is a test that by introducing contrast into the corpora cavernosa allows determining the integrity of the albuginea and corpora cavernosa, or quantifies the magnitude of the lesion.
If urethral damage is suspected, an exploration with contrast should be performed to check its status (urethrography).

Treatment of penile fracture
It is important to see a urologist immediately. This problem requires surgical treatment in most cases, to suture the tear. However, it can be treated conservatively, with rest, anti-inflammatory medications and ice bags (always covered with a cloth before applying them to the skin, to avoid burns), although the possibility of sequelae (curvatures, erectile dysfunction, etc.) is greater. When the urethra does not present damage, any accumulation of blood (hematoma) is drained and the damaged tissue of the corpus cavernosum and tunica is repaired. Surgical treatment produces satisfactory results in about 80% of patients; in the rest, residual pain and deformities may cause difficulties during sexual relations. All urethral lesions must be repaired, although urethral stenosis (narrowing) occurs in 20% of cases long-term after surgery.
Penis Amputation
The traumatic amputation of a part or the entire penis is often related to psychiatric disorders, fits of rage or jealousy. Bleeding can be significant, putting life at risk, especially when the amputation occurs with an erect penis. The goal of surgery is to restore the length of the member and its various functions. If we are faced with a complete section of the penis, it is advisable to take into account that proper preservation of the amputated organ (cleaning, placement in a sterile plastic bag and wrapping in ice) can make reimplantation viable even 24 hours after the event, with the best results achieved through microsurgical techniques.
