Why does our expression when we reach orgasm seem like pain? 'Pain and pleasure share a curious similarity in facial expressions', assures a book about orgasm. This close association has some interesting consequences. For example, the positive side is that during vaginal self-stimulation-induced orgasm, women are half as sensitive to pain as they usually are when in a state of rest.

In some cases, surgeons were asked to treat uncontrolled pain by cutting certain spinal cord nerve pathways in patients. However, this resulted in blocking the ability to have orgasms. Sometimes surgery had only temporary effectiveness, and the pain would return after six months. But so did the ability to have orgasms!The analysis of brain activity during orgasm and experimentally induced pain shows that at least two brain regions, the insular cortex and the anterior cingulate cortex, remain active during both experiences. This finding leads us to formulate interesting questions still without answers: How exactly does the brain distinguish between pain and pleasure? What is the difference between neurons that create the sensation of pain and those that create the sensation of orgasm? Could it be that the two brain regions possess a certain property common to both pleasure and pain, perhaps intense emotional expression but allowing for separation of different sensations of pain and pleasure? It seems plausible.Perhaps the pathways that transport sensations of pleasure and pain through the spinal cord and cerebral flow go together, so they produce similar effects of excitement and facial expression before branching off somewhere in the brain to pass the message or else to a zone of the pain-sensitive cortex or else to another zone of the pleasure-sensitive cortex. Until this bifurcation takes place, it is possible that sensory activity resulting from genital stimulation or a painful situation passes to the part of the brain where facial expression is generated, so that the resulting facial expression is curiously similar under both situations.
